This Season, we have a much simpler and quicker way for our members to keep the membership database up to date. The form below can be used to enter your contact/medical/playing details into the database.
Please note, that any date on the form needs to be entered in the format MM/DD/YYYY, so the 2nd of August 2007 would be entered as 08/02/2007.
Please fill in as much info as possible and please fill it in as soon as possible! If any of your details change, re-submit the complete form and your old info will be overwritten.

For U18 members only, you must submit a declaration (signed by a parent/guardian). This is in addition to the membership details form below. You can download this declaration here
This form has been designed to collect information on members taking part in activities organised by Stourport Hockey Club Ltd,(SHC). The information will be used for the general running of the club and the information supplied herein will only be distributed on a need to know basis. The Executive Committee, coaching personnel and Team Captains will have access to this information. The information will NOT be disseminated to anyone else without your express permission. SHC takes its responsibilities under the Data Protection Act 1998 very seriously. The Club and its representatives will not pass on any personal information collected through these forms without your consent unless required for genuine hockey reasons,emergency medical reasons or as required under the Data Protection Act 1998. We may, for example pass specific and limited personal details to facilitate an invitation by the schools and youth sections of Worcestershire Hockey Association or the Midlands Regional Hockey Association to selection trials. If any information provided through these forms changes, it is your responsibility to ensure that SHC is informed of any alterations as soon as possible.

I consider myself capable of taking part in hockey activities as organised and promoted by SHC. I have completed the medical details and consent that, in the event of any illness/injury/accident, any necessary treatment can be administered to me which may include the use of anaesthetics. I also understand that there is an inherent risk in participating in the sport of hockey and while coaches and team personnel will take every precaution to ensure that accidents do not happen, they cannot be held responsible for any loss, damageor injury suffered. I understand that it is my responsibility to inform a club representative if any of the details on this form change. By completing this form, I indicate my agreement with the statement above and also with the relevant Codes of Conducts listed below.
