Stourport Hockey Club

Registration with the Club is a means for us to obtain the necessary contact details to effectively communicate with you and a way for you to supply the important information that the Club needs to maintain to comply with England Hockey guidelines, and also to continue to supply the high duty of care that our members deserve.
If you are a VP, LIfe Member, Patron Member or Umpire, then registration grants you full membership for the current season as you are exempt from subscriptions.
If you are a not in one of the categories above, you MUST pay your subscription in order to become a member.
Once you have registered, the following will happen:
You will receive a confirmation email with your password, (you choose your username yourself during registration).
You will be directed to the subscription payment screen, (unless you are exempt from Subscriptions).
You will be prompted for your username and password in order to have access to the payment screen. Log in using the details that were emailed to you, and you will then be able to choose your subscription type and make the payment.
Once payment has been received, you will have member privileges and will be able to access member only content. Note, this includes teamsheets!

Paying your subscriptions early will yield a significant benefit, not only to you, but also to the Club.
Not paying your subscriptions will mean you are not a member of the Club and you are not eligible for selection. Furthermore, you will not be covered by the Club's third party insurance policy.
As a final incentive, subscriptions will increase by a MINIMUM of 15% after October 31st 2011.

In order to register, you will be asked to indicate your agreement with the statement below and also with the relevant Codes of Conducts listed below.
I consider myself capable of taking part in hockey activities as organised and promoted by SHC. I have completed the medical details and consent that, in the event of any illness/injury/accident, any necessary treatment can be administered to me which may include the use of anaesthetics. I also understand that there is an inherent risk in participating in the sport of hockey and while coaches and team personnel will take every precaution to ensure that accidents do not happen, they cannot be held responsible for any loss, damage or injury suffered. I understand that it is my responsibility to inform a club representative if any of the details on this form change.

This form has been designed to collect information on members taking part in activities organised by Stourport Hockey Club Ltd, (SHC). The information will be used for the general running of the club and the information supplied herein will only be distributed on a need to know basis. The Executive Committee, coaching personnel and Team Captains will have access to this information. The information will NOT be disseminated to anyone else without your express permission. SHC takes its responsibilities under the Data Protection Act 1998 very seriously. SHC and its representatives will not pass on any personal information collected through these forms without your consent unless required for genuine hockey reasons, emergency medical reasons or as required under the Data Protection Act 1998. We may, for example pass specific and limited personal details to facilitate an invitation by the schools and youth sections of Worcestershire Hockey Association or the Midlands Regional Hockey Association to selection trials. If any information provided through these forms changes, it is your responsibility to ensure that SHC is informed of any alterations as soon as possible.
